We provide continuing comprehensive health care for individuals up to age 24. Our overall goal is to provide high quality health care and excellent customer service to the community through our team of professional staff, as well as the additional resources of our Community Health Team. The care team gives patients and family access to evidence-based care and self-management support.

We offer sick visits by appointment but ask that patients call and speak with a nurse first so we can schedule an appropriate visit.

Outside of our regular office hours, there is always a provider on call to address your concerns.

Free services available in our office include certified lactation counseling and access to our Community Health Team, which is comprised of a registered dietician, a behavioral health counselor and a care coordinator. Please ask your provider how to access these services.

Remember to bring in your most up-to-date insurance card and photo ID. Please note that all co-pays are expected at the time of service. If you are unable to pay, Porter Medical Center offers financial assistance. For more information, call patient financial services at (802) 388-8808.

If you have questions or concerns, we hope you will call us. Our nurses have a wealth of knowledge and are always ready to advise and reassure worried parents.
